Cannot Delete A Revision In A Workflow.

Symptoms
You have a content item that has entered workflow "Miscellaneous". The author determined it had errors and tried to delete it before it was approved. This gave an error, which is expected since one cannot delete an item in an active workflow.
As sysadmin, you then tried to "force approve" the item. This seemed to work: the history shows the item was marked as approved by the user, but it does not release the item. When trying a second time to force approval, you get an error message similar to the following
Event generated by user 'sysadmin' at host 'xxx.xxx.com'. Unable to delete revision 20 for '11111'. Content item is part of workflow Miscellaneous. User has already acted on this content item for this step. [ Details ]

This error also occurs if you try to delete the item via its Info page, or the Repo Man applet. Both of these show the item status as REVIEW.
