Error WSM-00069 "The security header is missing" When Testing a Web Service in Fusion Middleware Control

Symptoms
A Web Service is secured with an OWSM service policy. When the Test Web Service screen in the Fusion Middleware Control (EM) is used to test the web service, it fails, even though the correct client policy is attached to the request. The following message is shown:
oracle.wsm.security.SecurityException: WSM-00069 : The security header is missing. Ensure that there is a valid security policy attached at the client side, and the policy is enabled.
The error occurs even though all necessary fields associated with the policy have been populated correctly. The same service works when accessed from other clients.
