Symptoms
OHS was placed in front of OAM. LBR setting was changed in OAM as shown below
Access Manager Settings for Load Balancing:
OAM Server: <OHS hostname>
OAM Server Port: <ohs port>
When accessing "http://<ohs hostname>:<ohs port>/myapp" from the browser the Custom Login Form is not displayed correctly, only the html code is shown in the browser
