File Upload Failing In Contributor UI Asset Form (Doc ID 1587089.1)

Symptoms

A user is unable to upload files in the Contributor UI asset form.  Other users (ie in india) are able to use the same asset form to upload files.

This has been reproduced in Win7/Firefox 23.0.1:

1. Open a contributor form
2. Fill in the values
3. Click on browse button
the upload bar goes till 100% and then "Upload Fails." pop up error message
