Symptoms

Using the DB Adapter to perform a merge on a many-to-many relationship. In this case the existence check is failing and insert is happening in place of update.

When using the DB Adapter to insert records into a many to many relationship table, seems to be updating one of the tables instead of the Record table intended for, therefore fails with a PK constraint violation.
