AQ Adapter Outbound Connections not Released after VM Crash (Doc ID 1613300.1)

Goal

We have 2 SOA machines in a cluster and 1 of the SOA machines (VM) crashed.  After recovering the machine, the Admin Server was restarted, SOA services (on both nodes) and all services seemed to be running fine.
But after a few days it was found that Datasouces and AQ Adapter outbound connections were not been released after the server crash. There were a lot of <Unavailable> outbound connectiona in the AQ Adapter, and the below error in the log:

Internal error: Cannot obtain XAConnection weblogic.common.ResourceException: No good connections available.
