Application Stripe List in <adminserver>/em is not Populated with "capture" or other Application Stripe Values (Doc ID 1618273.1)

Symptoms

Application Stripes, including the "capture" are not available in the Application Roles section of the <adminserver>:<adminport>/em page.  The "capture" Application Stripe is required for the post-configuration steps in the Enterprise Deployment Guide for WebCenter Enterprise Capture 11.1.1.8.  Even after restarting, the "capture" Managed Server, the Application Stripe does not appear.
