Title Not Defined Error when Checking In Document into a Folder from Site Studio Designer (Doc ID 1619548.1)

Symptoms

Unable to check-in Site Artifacts like element definitions, region definitions, etc from Site Studio Designer.  Even though title is given, the error says that title is not defined. This only happens if a Contribution Folder is defined in the Check-in Form from Site Studio Designer.  Without specifying contribution folder the check-in works fine.
The following error is seen in logs:
