Top Header Text of the Managed Attachment Dialog is not Displaying the Customer Name and Customer Number (Doc ID 1624660.1)

Symptoms

Having issues passing parameters from Oracle EBS to UCM using Managed Attachments functionality.

EBS OAF work fine for both of these scenarios.  It is only in EBS Forms where the problem occurs.

Essentially, When the Logon to EBS Forms – Receivables Manager – Transactions, -- a customer ticket is queried, then they click on the Zoom button.
Then, the Managed Attachments Content Server dialog can be launched and functions Normally.
--> In addition, as the important distinction:  The top header text of the Managed attachment dialog will display this:  "Managaed Attachments - Customer Name, Customer #"  (the name and # are derived from the original EBS Form.
This Form functions correctly with Managed Attachments.

When the Logon to EBS Forms –Service – Service Requests  -- a customer ticket is queried, then they click on the Zoom button.
Then, the Managed Attachments Content Server dialog can be launched and ALSO functions Normally!

Issue:  The top header text of the Managed attachment dialog  will display this:  "Managaed Attachments"  ==> The name and # are "missing!
This makes it hard to correlate the Customer to the particular Managed Attachment dialog.
