Replicat Abends With Errors OGG-01296 And OGG-01154 When Replicating to SQL*Server

Symptoms
You are using GoldenGate for SQL*Server -
Version 12.1.2.0.1 17597485 OGGCORE_12.1.2.0.T2_PLATFORMS_131206.0309
to replicate between 2 SQL*Server databases.
The replicat abends with the following error -
2014-03-08 09:12:30 WARNING OGG-01154 Oracle GoldenGate Delivery for SQL Server, <REPLICAT_NAME>.prm: SQL error 100 mapping <USER>.<TABLE> to <USER>.<TABLE> [SQL error 100 (0x64)]No data found.
2014-03-08 09:12:30 WARNING OGG-01003 Oracle GoldenGate Delivery for SQL Server, <REPLICAT_NAME>.prm: Repositioning to rba 38314629 in seqno 17.
2014-03-08 09:12:30 ERROR OGG-01296 Oracle GoldenGate Delivery for SQL Server, <REPLICAT_NAME>.prm: Error mapping from <USER>.<TABLE> to <USER>.<TABLE>.
2014-03-08 09:12:30 ERROR OGG-01668 Oracle GoldenGate Delivery for SQL Server, <REPLICAT_NAME>.prm: PROCESS ABENDING.
