Cannot Create A New Version Of OID Process Form Using Installed OID 11.1.1.6 Connector Either Using UI Or Design Console (Doc ID 1669136.1)

Goal

Recently installed OIM 11.1.2.1 and the OID connector 11.1.1.6 and trying to create a new version of the form via the UI. Then attempted to use design console to create a new version of the OID form and got the following errors:
