Symptoms
Error :
when trying to start a Forms Application after creating a new Forms WLS managed server or installing WLS_FORMS as a Windows Service.
Changes
New Forms WLS managed server created using <Note 989118.1> (WLS_MY_FORMS for example )
or
WLS managed server WLS_FORMS has been configured as a Windows Service.
