Blank Page on Remote Satellite Server with Error in Logs: "Failed to get user credentials for fwadmin from database" (Doc ID 1942315.1)

Symptoms

When trying to browse the website through Remote Satellite Server a blank page or 403 response is shown.

Remote Satellite Server log shows an error:
[2014-09-17 08:54:15.501][com.fatwire.wem.sso.oam.filter.OAMFilter][WARN] Failed to get user credentials for {user} from database

Sites log shows exception:
[2014-09-15 20:49:50,321 GMT+10:00] [ERROR] [.kernel.Default (self-tuning)'] [fatwire.logging.cs.auth] Login failed. Exception thrown while getting Dir context for user:ContentServer
javax.naming.AuthenticationException: [LDAP: error code 49 - Invalid Credentials]

Changes

The delivery WebCenter Sites environment and Remote Satellite are integrated with OAM and Sites is integrated with LDAP.

Remote Satellite Server pages are protected through OAM.

Note: the documentation assumes only the WebCenter Sites UI needs to be protected and Remote Satellite Server pages are unprotected. It is when Remote Satellite Server pages are protected that this problem occurs.
