Batchloader Throws Primary File Not Found Error

Symptoms
Trying to use batchloader to import documents into a newly installed systems. The content being added has multiple revisions. After the first revision checked in, batchloader throws error saying the primary file not found for the next release. Though the file is there.
The batchloader log file shows an error like this:
