Unable To Login To BPM Workspace Or Composer Due To Error: "oracle.wsm.policymanager.PolicyManagerException: WSM-02141 : Unable to connect to the policy access service."

This is a new install of SOA 12.1.3 and OSB 12. It worked fine for a period of time.
All of a sudden, the users can not login to either the composer or workspace.
The wsm-pm application is working fine; however the policy manager is not working since the wsm-pm validator can not display the policies.
The error message seen in the log file is:
"oracle.wsm.policymanager.PolicyManagerException: WSM-02141 : Unable to connect to the policy access service."
The issue can be reproduced at will with the following steps:
1. Try to login to either the composer or workspace
2. Check the log file
3. Check the wsm-pm validator. It will generate this error:
The issue has the following business impact:
Because of this issue, users can not login to the composer or workspace.
