FMW 11g Patch Set ApplySession Failed Because of OPatch Problem Requesting emocmrsp Run for OCM

When attempting to apply a new FMW Patch Set release on a previously existing FMW installation, even after applying patch successfully following errors are seen in the install logs:


OPatch failed with error code 25]]]
[2015-04-20T10:58:45.664+08:00] [as] [NOTIFICATION] [] [] [tid: 15] [ecid: 0000KnKjvcOEwGK5qVT4iY1LD6bk000005,0] stderr=[[ Error during Oracle Home discovery Phase]. Detail: OPatch failed: ApplySession failed to prepare the system. [[To run in silent mode, OPatch requires a response file for Oracle Configuration Manager (OCM). Please run "/u01/app/oracle/middleware/oracle_WT/OPatch/ocm/bin/emocmrsp" to generate an OCM response file. The generated response filecan be reused on different platforms and in multiple OPatch silent installs.
