Reverting from Apex 5.0 to 4.0.2 Results in No Apex in DBA_REGISTRY (Doc ID 2031205.1)

Symptoms

 1.  Followed the steps in the documentation to revert APEX 5.0 to APEX 4.0.

 2. After completion there is no APEX version registered in the DBA_REGISTRY. 

           The apex_verify script returns "APEX IS NOT INSTALLED"

      and

            APEX do not show if one query from DBA_REGISTRY

Changes

 Upgraded APEX 4.0 to 5.0 and then tried to revert to APEX 4.0.
