Vanity URL Not Working on Delivery (Doc ID 2041098.1)

Symptoms

Assets are published from Management to Delivery, but when attempting to view the asset on Delivery using its vanity URL, it shows "Content Server could not process the request" and returns a 404 status.

The sites.log shows:

[2015-07-29 11:13:21,372 UTC] [WARN ] [.kernel.Default (self-tuning)'] [com.fatwire.logging.cs] ContentServer attempted to invoke a nested page that does not exist: Unable to locate page MySite/MyAssetType/MyTemplate
