Cannot login to Sites, logging shows that login succeeds but soon afterwards the user becomes DefaultReader again

Login to Sites UI fails with
Not Authorized
This account is not authorized to access this site.
To gain access to the system, you will need to login with an account authorized to access this site.
Sites.log does not show any errors when logging in, but com.fatwire.logging.cs.auth=DEBUG shows a fallback to DefaultReader taking place after login:
[2015-08-19 10:03:20,388 WEST] [DEBUG] [.kernel.Default (self-tuning)'] [fatwire.logging.cs.auth] Logged in user DefaultReader with ACLs Browser,Visitor and timeout 91800
Also, other means of login also fail such as SitesExplorer, and CatalogMover. In all cases there is no error, but the message "Logged in user DefaultReader" is seen soon after login.
It is confirmed that Sites is configured correctly:
- User exists with the correct name and password, roles and ACLs
- Correct DB schema is being used
- LDAP is not enabled (or is enabled correctly)
