Monitor V12.1.3 UI Page Is Blank After Logon. Monitor Log Shows Java.lang.IllegalStateException: Response Already Committed (Doc ID 2058407.1)

Symptoms

 After a fresh install of Monitor v12.1.3, creating a new Monitor userID and logging into Monitor web, the Monitor UI is blank. The MONITORSERVER_server1-diagnostic.log is showing:
