Symptoms
Anytime a date is placed in a Forms text item of Data Type Datetime - whether this information comes from the DB or sourced from a non-DB Forms text field - an internal "assumption" is made that the sourced information is in GMT and an implicit conversion is made to the local time zone.

Upgraded to Forms 12c. Here is an example of what will be seen when a client is -7 from GMT:
