Custom Application Receiving Intermittent Error from OID 11g 11.1.1.7: LDAP: error code 19 - Attribute Uniqueness Constraint Violation

Symptoms
Oracle Intenet Directory (OID) 11g 11.1.1.7.
This was working before.
Using a java background custom code with DirContext for adding users to OID (e.g., ldapContext.createSubcontext(dn,userAttributes);).
Intermittently receiving the error message below in the custom application from OID:
LDAP: error code 19 - Attribute Uniqueness Constraint Violation
The failures are not consistent, for example if sending 100 requests, about ~5 to 10 can fail.
Using the same values as reported in one of the failures via ldapadd command line is successful.
