Application Instance Form Fields Are Not Following the Order Of the Fields in Defined in Its Process Form

Symptoms
The order of the fields seen in Application Instance form is not following the order of the fields defined in its backend process form.
For example In Development environment the fields on the RACF application instance form appear to be displayed based on the order value associated with each field on the process form. But in Acceptance environment, the same form fields appears to be displayed based in the alphabetical order of "Field Name" and not by the order value specified in the process form.
EXPECTED BEHAVIOR

The order of the fields should remain the same across various environments when the same application instance resource and process form is being used.
STEPS
--------------
The issue can be reproduced using the following steps:
1. Provision the RACF application to a user.
2. Once provisioned, review the application instance form after selecting the application instance in the User's Account tab.
3. Notice the order of the fields shown UI. If the issue happens, the order of the forms will not be per the Order defined in the Process form.
