ODI 12c Load Plan Gets Corrupted when Editing Any Of Its Steps and Causes Execution Failures (Doc ID 2162659.1)

One of the following symptoms is observed when editing or executing an Oracle Data Integrator (ODI) 12.1.3 Load Plan (LP):

Violation of AK_LP_STEP_GUID constraint when adding steps to the Load Plan...

When trying to add a few steps to the LP, and save it, the following error occurs:
LP steps are not properly deleted from the database when they are deleted in the ODI Studio UI...
More precisely, the column PAR_I_LP_STEP (in the Work Repository SNP_LP_STEP table) is set to NULL, and then the LP cannot be executed anymore (will stay in **waiting** status indefinitely).
