PSA schema upgrade from 11.1.1.6 to 11.1.1.9 fails with error, 'UPGAST-00221' (Doc ID 2169136.1)

Symptoms

 When attempting to upgrade the SOAINFRA schema using the Patch Set Assistant it fails with an error in the PSA log, "UPGAST-00221: unexpected error uprading schema".
