Symptoms

Running the 12c Upgrade Assistant (ua.bat or ua.sh), in Step 8 of 11 of the Oracle Fusion Middleware Upgrade Guide, you get the a red X next to one of the Component Names and a red failed status on Oracle Platform Security Services:
