Symptoms
Dependent option lists are not working when there are two or more children fields that depend on the same parent field and one of those children fields is excluded, hidden or uneditable in the physical profile. If all the children fields are available or required, then all fields are populated properly.
This is a known WCC core issue that is affecting WCCREC/URM Simple profiles for electronic and physical items. This issue is described in greater detail within the KM note below.

Changes
The option lists/DCL's were working fine previously in 11.1.1.9 until WCC/WCCREC 11.1.1.9 Bundle Patch (between BP 151221 and BP 160829) was applied. This issue is also seen in 12.2.1.2.0 and 12.2.1.2.171218.
