OIM Sending Duplicate Email Notifications For All Enabled Notifications and users

Information in this document applies to any platform.
OIM is sending out duplicate emails whenever a Notification is sent.
For instance, we send out emails on Self Service Registration Account creation, temporary password generation, etc.
Whenever a notification event occurs two duplicate messages are sent to the user.
Examining the logs, the notification event only fires once, and the emails are identical.
If a new password is generated as part of the event, the password in each email is identical.
This Proves that the event is only firing once, but two emails are sent.
