OUD 11g - Indexes are Corrupt after Upgrading from PS2 -> PS3 (Doc ID 2234698.1)

Symptoms

After upgrading from OUD 11.1.2.2 -> OUD 11.1.2.3, the following errors were encountered:

. This value seems to be corrupt . The index should be checked or rebuilt
[07/Oct/2016:15:58:54 -0600] category=JEB severity=SEVERE_ERROR msgID=8650998 msg=An error occurred while reading from index <suffix>_telephoneNumber.substring <key>
. This value seems to be corrupt . The index should be checked or rebuilt
