Incorrect Disconnect App Instance And Disconnected Flat File App Instance Request Status When Enable Or Disable Of Account Or Revoke Entitlement (Doc ID 2244372.1)

Symptoms

When Disabling or Enabling a Disconnected Flat File App Instance Account (or Disconnected App Instance), after approving the child default operational approval request, the parent request goes into "Request Completed" but the child fulfillment task which is created correctly goes into an "Assigned" state.

The parent approval request should have gone into "Request Approved Fulfillment Pending" as it does during a provision and revoke.

Additionally, when an end user requests an entitlement revocation for a Disconnected Flat File Application Instances (or Disconnected App Instance) and the request is approved by an admin (i.e. xelsysadm) then the request goes to "Request Completed" even though a newly assigned manual fulfillment task was created.

The parent request status should have gone to "Request Approved Fulfillment Pending".
