Form Not Displaying For Application Instance Request For Multiple Users

Information in this document applies to any platform.
When raising a request for an application instance for multiple users in the same request, there is no form shown.
From Self-Service > Request Access > Request For Others > Add at least 2 users > Next > Select Application Radio Button > Search The application Instance > Add To Cart > Next > Click on the edit pencil tab under Request Details section and there is no application form shown.
