Form Not Displaying For Application Instance Request For Multiple Users

When raising a request for an application instance for multiple users in the same request, there is no form shown (application instance from which was created from process form)
Login to self-service > Request Access > Request For Others > Add atleast 2/3 users > Next > Select Application Radio Button > Search The application Instance > Add To Cart > Next > Click on the edit pencil tab under Request Details section and there is no application form shown.
