Oracle Access Manager 11g R2PS3 (OAM 11.1.2.3): Requested URL /oam/server/auth_cred_submit Was Not Found (Doc ID 2258836.1)

Symptoms

Authentication via DCC result in http 404 for /oam/server/auth_cred_submit For second login after first login/logout.

This is seen on  Safari and browsers on  Apple/windows devices.

Below are the steps to reproduce

1. Login to OAM protected page using DCC
2. Logout will redirect back to Login page
3. Login again

Users gets The Requested URL /oam/server/auth_cred_submit was not found
