Incoming Requests to OHS from F5 LoadBalancer are Not Responding (Doc ID 2259168.1)

Symptoms

An application is deployed to a WebLogic Cluster or Managed Server using the following typology with a F5 Load Balancer (LBR) -- where SSL is terminated:

Errors do not exist in the Oracle HTTP Server ohs1 error_log (or ohs1.log).  Located at $INSTANCE_HOME/diagnostics/logs/OHS/ohs1.

No subsequent Oracle HTTP Server access_log entries exist from the F5 LBR request.
