Requests Immediately Go To "Request Failed" State After Submission. (Doc ID 2280547.1)

Symptoms

Directly after submitting the request, the request immediately goes to a "Request Failed" state.  When opening the request to view the details an error popup is seen with:

JBO-29000: Unexpected exception caught: oracle.bpel.services.workflow.client.WorkflowServiceClientException, msg=javax.naming.NameNotFoundException: Unable to resolve 'TaskQueryService'. Resolved '' [Root exception is javax.naming.NameNotFoundException: Unable to resolve 'TaskQueryService'. Resolved '']; remaining name 'TaskQueryService'
