Page Not Found error appears in the browser window when attempting to run a form
via 9iAS Rel 2 Forms Services e.g 
The URLs may look like:

 http://<hostname.domain>:port/forms90/f90servlet?form=test.fmx  

 http://<hostname.domain>:port/forms90/f90servlet?form=test.fmx
