Symptoms

1. Using APEX 5.0.1 with ORDS 3.0.11 deployed in Tomcat 7.0.63.

2. PDF Formatting was setup using steps from the technical brief on customizing pdf reports.

3.  After upgrading from ORDS 2.0.9 to ORDS 3.0.11, the pdf report footers are missing and the headers are truncated.
