Symptoms
When using the EZCONNECT or the HOSTNAME naming adapter to connect to Forms 12C, a blank error pop-up is displayed and the connection fails. The problem did not occur in earlier versions.
The standard notation for EZCONNECT follows this convention:
username/password@hostname:port/service_name
