Document is no Longer Viewable in Content UI (ADF) after Adding Annotation (Doc ID 2314661.1)

Symptoms

Added annotation to the document.  The document viewer displays the document and annotation, but the Annotation section doesn't appear in the Summary tab.  The Annotation section is normally under the Renditions section and contains Annotated PDF/TIFF links.

 

Clicked on ADFUI refresh button, but the Annotation section still doesn't appear.  Clicked on browser refresh, but only the annotation was visible.

 

Clicked on browser refresh again, but neither the document or annotation is displayed.  Only a black rectangle is displayed and sometimes a yellow rectangle is also seen.  Tried closing the document viewer tab and opening the document, but the document is still not viewable.  Other users are not able to view it either.

 

Checked the annotationRendition.xml file in the weblayout directory and it was incomplete.

 Added annotation to the document in Content UI
