Oracle Access Manager 11gr2 (OAM 11.1.2.3.0) FED-15035 Provider ID is Missing from the Authentication Request

Federation fails with "FED-15035 Provider ID is missing from the authentication request" when OAM id functioning as IdP when the SP partner sends the following in the authentication request with an otherwise apparently valid Provider ID value:
