Managed Server Not Able to Connect to the AdminServer Over Admin Port and Getting FAILED

Symptoms

Managed server instance cannot reach admin instance during start up (after OS patching) and fails with the following errors :

<Oct 22, 2017 5:43:02 PM CDT> <Error> <Security> <BEA-090870> <The realm "myrealm" failed to be loaded: weblogic.security.service.SecurityServiceException: com.bea.common.engine.ServiceInitializationException: weblogic.security.spi.ProviderInitializationException: A failure occurred attempting to load LDIF for provider Authorizer from file ${MW_HOME_12.1.1}/wlserver_12.1/server/lib/XACMLAuthorizerInit.ldift
