When upgrading Forms and Reports from 11g to 12.2.1, it fails at the domain reconfiguration step with below errors:

CFGFWK-64254: An occurred in "validating Domain" phase execution

CFGFWK-60953: An error in the reconfiguration templates resulted in the following deployments being left in the original MW home

Note: This applies to all Fusion Middleware 11g to 12c upgrades, not just Forms and Reports.
