You are configuring OMS to monitor virtual Exalogic Rack by referring official document:

At step '2.3.2.3 Adding Oracle ZFS Storage Appliance Instances for Monitoring', user 'oracle_agent' is failed to connect Oracle ZFS Storage Appliance instances even though username and password is correct.
