ADF Rich Text Editor Not Working Properly When Trying To Select Bold, Italic, Underline

Symptoms
Rich Text Editor (RTE) doesn't behaves correctly when user tries to select font text formatting for Bold, Italic, Underline. The following unexpected behaviors have been observed:
Behavior #1:
Cannot select Bold and Italic and Underline.
Select Bold and then select Italic but this causes Bold being deselected. If now you select Underline this causes Italic being deselected.
Behavior #2:
Type something, select Italic, type, select Bold, deselect Italic, this causes Bold and Italic being deselected.
None of these behaviors are seen in 12.2.1.2.0.
