RSA plugin always exists on PAUSE instead of FAILURE (Doc ID 2390999.1)

Symptoms

  1. The RSAPlugin's behavior when an invalid PIN/Passcode is entered by the user is to always exit on PAUSE state instead of FAILURE.
  2. When user entered incorrect username with valid password, user is taken in to login page as expected. Now if user enters valid user name and password, OAM does not allow them to login.
