Symptoms

Oracle Access Manager 12c PS3 (OAM 12.2.1.3)-OIM Integrated environment:  First Time User Login throws : OAM System Error

Steps
OAM/OIM 12c has been integrated.
User is successfully created in ID Store OUD by OIM.
User logs in for the first time
User is challenged to change password/password reset for the first time login.
User is also asked to set Password Challenge Questions.
After User password reset and challenge questions set, the User is seeing OAM System Error
