User Profile Changes are not Propagated to Accounts. ERROR:"IAM-0089999" or "Error occurred while propagating user changes" with "NullPointerException"

Symptoms
While attempting to modify multiple attributes for a user with multiple accounts, the profile changes are not propagated to the target accounts.
The UsrProcessTrigger process task is not available in the task history table.
The following errors/warnings are seen:
The issue can be reproduced with the following steps:
1. For a User/Identity with multiple (10+) accounts provisioned.
2. Open "Modify User" form and update more than one attribute (for example, Timezone and Currency).
3. Observe the errors indicated above.
NOTE: This issue is specific to the "Bulk Attribute Propagation" feature as per KM <Document 1595938.1>

A patch has recently been applied which contains the fix for <Bug 23110063>.
