SPML-DSML Not Getting Deployed In OIM 12c And Landing In "Prepared" Status After Deployment (Doc ID 2426758.1)

Symptoms

OIM 12c, Microsoft Active Directory Password Sync Connector 9.1.1.5

Trying to deploy SPML-DSML for OIM-AD Password Synch install and configuration but SPML-DSML is not deployable  

Trying to install Active Directory Password Synch connector for OIM

But can't deploy the SPML-DSML in OIM

 

Also finding that even after successfully deploying SPML-DSML, the Enterprise Application is landing into "Prepared" status and not "Active" status:

 

Also after above attempts the SMPL_DSML appears as active for testing the service with this address: http: //< HOST>: <NON-SSL_PORT> / spmlws / OIMProvisioning but it does not load the "spmlws" and throws following error:
