APEX: Unable To Use The Sub Template .xsl When Calling From BI Publisher (Doc ID 2439260.1)

Symptoms

When using a BI Pub .rtf template sub-templates with a  with APEX the request fails with:

Caused by: java.util.EmptyStackException

This error initially appeared in BI Publisher, and was resolved with the solution in:
<Note 2247221.1> - BI Publisher Desktop Templates Fail With Error 'oracle.xdo.XDOException: java.util.EmptyStackException' When Using Word Plugin 12.2.1.2 (64bit) With Microsoft Office 2016 (64bit) - was implemented but the error still occurs.
