Forgot User Login Throws "A system error has occurred.." With ChallengePolicyDisableException Due to "PCQ.FORCE_SET_QUES" System Property Check

Symptoms
Forgot User Login? option throws "A system error has occurred. Please contact your System Administrator." error upon giving the user's email id.
This issue can be reproduced with the below steps :
1. Access OIM identity console.
2. Click on Login
3. Click on Forgot User Login? option
4. Enter the valid email id of the user and click on Submit.
5. One is shown below error on console:
"A system error has occurred. Please contact your System Administrator."
6. No email sent to user .
7. OIM logs shows the below errors .
Caused By: oracle.iam.selfservice.exception.ChallengePolicyDisableException: Challenge policy is disabled for the user.
