Forms using FSAL Mode Fails to Start after Patch 28044002 - signer information does not match signer information of other classes in the same package (Doc ID 2491578.1)

Symptoms

Forms Standalone Launcher (FSAL) won't start after applying <Patch 28044002>.  The following message occurs in the java console output.

at java.lang.reflect.Method.invoke(Method.java:498)
at oracle.forms.saa.FormsStandAloneApp.main(Unknown Source)
Caused by: java.lang.SecurityException: class "oracle.forms.engine.Main"'s signer information does not match signer information of other classes in the same package

 Upgraded to Forms 12.2.1.3 and applied <Patch 28044002>.
